U.S. statistics project that the population of people 65 years and older is estimated to increase by 42% between 2019 and 2034 (according to an Association of American Medical Colleges (AAMC) June 2021 publication). A significant percentage of Medicare beneficiaries live with at least one chronic condition, while 66% live with two or more chronic conditions [1].
It is estimated that Adverse Drug Events (ADE) cost a health system an additional $8,000 and $25,000 per patient. According to American Pharmacists Association, approximately 1.5 million preventable ADE’s occur each year resulting in $177 billion in injury and death! ADE’s lead to increased mortality, hospitalization days, emergency room visits, and overall cost of care.
The path to prevention for patients with high-risk high-needs prescription requirements begins with enhancing the relationship between prescribing physicians and pharmacists. Causes if ADE’s include:
- Prescribing the wrong drug to patients who do not properly metabolize the drug
- Incorrect dosage based on patient metabolism
- Conflicts with other prescribed medications
- Nonadherence
- Cost
At PRM, we are working with industry experts to create a population-based assessment tool utilizing data analytics and artificial intelligence to pro-actively identify potential ADE events BEFORE they occur. This will help in revolutionizing the process of prevention and assist in the development of personalized medication care plans. Stay tuned for more information on this new service. #mtm
